By applying for a commercial line of credit from Voltage Credit, LLC (“Voltage Credit” or “we”), you acknowledge and agree that you are authorized to apply for a line of credit on behalf of your company. You also attest that the information you provided is true and correct to the best of your knowledge. Knowingly providing fraudulent information in connection with your application with the intent to commit fraud is a crime.

Important Information about Procedures for Opening a New Account

To help the government fight the funding of terrorism and money laundering activities, federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account. When you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.

Politically Exposed Parties

By submitting this application, you certify that no authorized signer, controlling person, or beneficial owner is a politically exposed person (PEPs). For the purposes of this application, a PEP is defined as any senior foreign political figure, foreign government official, foreign military official or a foreign government owned business entity, or related to or a close associate of a PEP. You agree to immediately notify us if you become aware that any authorized signer, controlling person, or beneficial owner is or becomes a PEP.

Consent to Credit Check

You hereby authorize Voltage Credit, LLC and its designated agents or representatives to obtain a consumer credit report and/or investigative consumer report about Applicant, its officers, directors, and/or its beneficial owners for the purpose of evaluating Applicant’s creditworthiness.  

You understand that this report may include information regarding your credit history, payment habits, and other financial information as permitted by law. This information may be obtained from credit bureaus, financial institutions, or other sources. 

 

You acknowledge that this authorization is voluntary, and that you have the right to request a copy of any credit report obtained about Applicant, as well as a summary of your rights under the Fair Credit Reporting Act (FCRA). You also understand that you may revoke this authorization in writing at any time, though such revocation will not affect actions taken prior to receipt of the revocation.

If your application for business credit is denied, you have the right to a written statement of the specific reasons for the denial. To obtain the statement, please contact Voltage Credit at voltagecredit@voltage.cloud or (785) 318-6521 within 60 days from the date you are notified of our decision. We will send you a written statement of reasons for the denial within 30 days of receiving your request for the statement.

We do not charge a fee for requesting a paper copy. Please allow us up to 30 days to respond to your request.
